Additional Information:

By checking this box, you authorize EA Quilting to perform Longarm services pertaining to the project and requests outlined in this form. Any changes made by the requestor after work on the outlined project has commenced, or past 48 hours after submission, will be done at your own additional expense. You understand the risks associated with machine quilting and that EA Quilting is not liable for any damages such as thread bleed, faulty piecing, and hidden flaws. Unclaimed items surpassing 60 days may be sold or disposed of to recuperate costs.
